Patient Centered Care

Patient-centered care is a model in which providers work closely with patients to meet the full range of patient needs and preferences.

Patient Centered Medical Homes (PCMHs) are health care settings that facilitate partnerships between individual patients, their primary care physicians, and when appropriate, the patient’s family. Previous research has shown that patient-centered medical homes have greatly improved patient care especially concerning those most in need of it.

Hackensack Alliance ACO offers practices in the ACO the resources to become certified as patient-centered medical homes. This recognition is awarded by the National Committee for Quality Assurance (NCQA), a non-profit organization dedicated to improving healthcare quality. Hackensack Alliance ACO believes this approach supports the highest level of care and improved patient outcomes.

Patient care is coordinated by electronic medical records, real time electronic reporting, registries, and other electronic resources to provide Medicare Fee-for-Service patients with appropriate care in a timely manner that suits their individual needs. The ACO is committed to having all its physicians using electronic medical records, electronic prescribing and other clinical information technology to help them provide the best care possible.
