Physician Participation Requirements

Board certification
Eligible to receive Primary Care Incentive Program payments from CMS
Electronic Medical Record (EMR must be eligible for incentive programs through the ONC)
Patient-Centered Medical Home (PCMH) certification within 12 months of joining; ACO provides consultant to assist with application and preparation
Commitment to the triple aim (improve experience of care, improve health of populations, lower per-capita cost of care)
Willingness to comply with the clinical and administrative policies of the ACO
