Transitioning from Pediatric Care to Adult Care
The transition from the pediatric to the adult world can be difficult for young people with diabetes. This has bee recognized among specialists in diabetes care for a long time. In order to ease the transition of a child or adolescent into the adult who is expected to be responsible for all aspects of his or her health care we offer a special program here at Hackensack University Medical Center.
Adolescents who are ready to transition are being encouraged to attend visits to the MOLLY Center for Children with Diabetes and Endocrine Disorders where they are seen by both pediatric and adult practitioners. Physicians, advanced practice nurses, registered nurses and registered dietitians who are certified diabetes educators are available. Patients receive assistance for the social and emotional aspects of diabetes from our social worker (s). Additionally, each patient is assessed for their readiness to move on to the adult world of health care.
Hackensack University Medical Center has a number of adult endocrinologists and diabetologists who can continue the same level of care started in the MOLLY Center for Children with Diabetes and Endocrine Disorders.
