Whether you are coming home from the hospital, or have been diagnosed with a chronic disease such as diabetes or COPD, having the support of a skilled nurse to help manage your condition can help you and your family gain confidence and independence. When your condition limits your mobility, your doctor may order home health care to provide medical treatment in your home, with the goal of helping you recover and regain your independence.
Home health care is often appropriate for elderly patients with the following conditions: congestive heart failure (CHF), COPD, diabetes, orthopedic recovery, stroke, chronic kidney disease, wound care, dementia and behavioral health needs. Home Health Care Team
It is important that you and your family trust the health care professionals you invite into your home. The nurses, therapists and other caregivers of Home Health Services of Hackensack provide experienced, compassionate health care and work closely with your physician to coordinate all aspects of your care. Care Transitions
Preparing for a successful transition from the hospital to your home is important to your recovery. That's why Home Health Services of Hackensack provides a care transitions program focused on getting you back home safely. Your care transitions coordinator will meet with you and your family to better understand how to take care of yourself when you get home, coordinate with the hospital and your primary care doctor, and ensure your medications are in order.
