With locations in Hackensack and Secaucus, Home Health Services of Hackensack has been selected as the preferred home health provider of Hackensack University Medical Center.

Whether you are coming home from the hospital, or have been diagnosed with a chronic disease such as diabetes or COPD, having the support of a skilled nurse to help manage your condition can help you and your family gain confidence and independence. When your condition limits your mobility, your doctor may order home health care to provide medical treatment in your home, with the goal of helping you recover and regain your independence.

Home health care is often appropriate for elderly patients with the following conditions: congestive heart failure (CHF), COPD, diabetes, orthopedic recovery, stroke, chronic kidney disease, wound care, dementia and behavioral health needs. Talk with your doctor if you think home health care may be right for you.

Home Health Care Team

It is important that you and your family trust the health care professionals you invite into your home. The nurses, therapists and other caregivers of Home Health Services of Hackensack provide experienced, compassionate health care and work closely with your physician to coordinate all aspects of your care. The care team includes:

  • RNs and LPNs to coordinate the plan of care your physician prescribes, monitor vital signs, administer medications, care for wounds, provide patient training, and other medical services
    • Specially trained nurses for congestive heart failure (CHF) recovery
    • Nurses certified in specialized care for wounds and ostomies
  • Physical therapists to help you recover strength, mobility and flexibility
  • Occupational and speech therapists to help you regain the ability to perform the tasks of independent living
  • Psychiatric nurses to help with conditions like dementia, Alzheimer's and depression
  • Social workers to help you with social and emotional concerns, including access to community resources
  • Certified home health aides to provide assistance with bathing, dressing and other personal care needs
  • Care transition coordinators to help you understand your condition and medications, and go home safely

An empowering approach is taken to help you and your family understand your condition, how to manage it and how to live your life to the fullest with a chronic disease or other health condition. Caring clinicians are trained to understand the whole patient — not just the medical diagnosis.

Care Transitions

Preparing for a successful transition from the hospital to your home is important to your recovery. That's why Home Health Services of Hackensack provides a care transitions program focused on getting you back home safely. Your care transitions coordinator will meet with you and your family to better understand how to take care of yourself when you get home, coordinate with the hospital and your primary care doctor, and ensure your medications are in order.
