The Center for Healthy Senior Living was recently awarded Patient Centered Medical Home level 3 status.

Recently recognized by the US News and World Report: Among Top 50 Geriatric Programs  for 8 yrs: The Geriatrics Program at HackensackUMC received a ranking of  #32 for 2011-2012.

In 2011 Geriatrics was also recognized by the Joint  Commission for disease-specific care certification and has earned the Joint Commission’s Gold Seal of Approval™ for its Geriatrics Program , this is the first certification in the country in Geriatrics.

The Center for Healthy Senior Living specializes in the evaluation and treatment of the ambulatory to the frail elderly. Board Certified geriatricians provide both a comprehensive assessment of an elder’s healthcare needs and a targeted program designed to address those needs.This includes vaccinations, screenings and assessment of cognitive and physical functional status, coordination of care including arrangement of appropriate blood work. Education for patient and caregivers including psychosocial, well being and ancillary support is provided. Transition of care and a focus on planning for the future are all part of the care for patients 65 and over.  We offer both a primary care service and a consultative program of coordination of care arranged in conjunction with an individual’s primary care physician.

An emphasis is placed on diagnosing illnesses and managing them in order to maximize function and quality of life for the longest period of time possible. Because each patient is unique, the purpose of a geriatric evaluation is to provide an individualized assessment and care plan. The physicians work closely with family members and caregivers to provide support and coordinate care when necessary. Our patients are evaluated for chronic conditions and geriatric syndromes (e.g. unsteady gait, incontinence, falls) by a geriatrician. Attention is also directed to a patient’s social situation. The center provides ongoing comprehensive primary care to all senior persons, taking into consideration physical function, the need for rehabilitation, and medication management. The physicians also offer caregiver education and materials about county resources for in-home support.
