Enlargement of the prostate, also known as benign prostatic hyperplasia (BPH), is common among men as they age. One-half of men over the age of 50 experience the symptoms of BPH, and 80 percent of men in their 80s have an enlarged prostate. BPH is a condition that is prevalent, yet also treatable.
Symptoms include weak urine flow, difficulty with initiating urine flow, frequency of urination, and waking up at night to urinate. Some men may have difficulty emptying their bladders, which may progress to the complete inability to urinate (urinary retention).
Evaluation
Diagnosis begins with a detailed history and physical examination and may include questionnaires, uroflowmetry, and urine studies. Some men may require further evaluation, which may include video urodynamics, cystoscopy, and/or prostate ultrasound.
Treatments Offered
Treatment is catered to each patient in a unique way depending on the evaluation. Treatment options include:
-
Transurethral Microwave Thermotherapy (TMMT): a minimally invasive approach to ablating growth of benign prostate tissue using microwave energy. This procedure is done in an office setting without the need for general anesthesia.
-
Transurethral Needle Ablation (TUNA): a minimally invasive approach to ablating growth of benign prostate tissue using radiofrequency energy. This procedure is done in an office setting without the need for general anesthesia.
-
Photoselective Vaporization of the Prostate (PVP): a minimally invasive outpatient approach to ablating/vaporizing growth of benign prostate tissue through the use of a laser. Urologists at Hackensack University Medical Center use the GreenLight™ laser system for PVP, which is as effective as surgical treatment (transurethral resection of the prostate, TURP) for BPH. During PVP, a small fiber is inserted into the urethra through a scope. The fiber carries laser energy, which quickly vaporizes and completely removes enlarged prostate tissue. Natural urine flow is rapidly restored and urinary symptoms relieved. PVP is virtually bloodless and reduces the side effects of pain and burning. It preserves sexual function and offers a quick return to normal activities. PVP is durable and offers long-lasting relief, often up to five years.
-
Transurethral Resection of the Prostate (TURP): the traditional surgical tissue
-
Open Prostatectomy: a surgical procedure for the treatment of BPH, which is reserved for extremely large prostates
